Yes, you have to have bare scalp to wear a vacuum hairpiece, for it to "stick on". The lace cap ones with silicone around the edges are good when you have a lot of hair loss or big patches.

I don't get injections anymore. My hair's back, thank goodness. My friend who lost all her hair and then it grew back got a more aggressive program of injections. I'd ask your dermatologist.
